Passports
The Collector's Office has been certified as a Passport Acceptance Agency!  Applications are being accepted during regular business hours in the Collector's Office, Room 4 in City Hall.

Effective Feb. 1, 2008 - All forms, fees and rules for children under age 16 have changed.  These new forms are available in the Collector's Office, and the fees and rules have been updated below.


NEW PASSPORT (Ages 16 and up)
you will need:
- 2 identical color passport photographs, 2"x2" with a white background (taken no more than 6 months ago)
- Proof of Citizenship (one of the following)
        Certified Birth Certificate
                (must show applicant's full name, date & place of birth, raised /embossed seal and file date)
        Expired US Passport
- Proof of Identity (one of the following)
        Driver's License
        Military ID
- Completed DS-11 form
- Check or Money Order made out to the US Dept of State
        $75 (regular service)                   Passport Processing Schedule
        $135 (expedited service)
- Check or Money Order made out to the City of Salem
        $25
        

NEW PASSPORT (Ages 15 and under)
you will need:
- 2 identical color passport photographs, 2"x2" with a white background (taken no more than 6 months ago)
- Proof of Citizenship (one of the following)
        Certified Birth Certificate
                (must show applicant's full name, date & place of birth, raised /embossed seal and file date.  
                Under age 16, parents' names must also appear on the certificate)
- Proof of Identity
        Ex: School ID
- Both parents and child must be present when submitting application.
        If only one parent appears, you must also submit one of the following:
                - Second parent's notarized written statement consenting to passport issuance for the child
                - Primary evidence of sole authority to apply
                - A written statement (made under penalty of perjury) explaining the second parent's unavailability)
- Completed DS-11 form
- Check or Money Order made out to the US Dept of State
        $60 (regular service)                   Passport Processing Schedule
        $120 (expedited service)
- Check or Money Order made out to the City of Salem
        $25



PASSPORT RENEWAL
You may use form DS-82 if:
- You can submit your most recent US Passport
- You were at least 16 years old when your most recent US Passport was issued
- You were issued your most recent US Passport less then 15 years ago
- You use the same name as on your most recent US Passport  OR  You have changed your name by marriage or court order and can submit proper documentation to reflect your name change

** If you are unable to meet all of the above specifications, you are considered a new applicant **

To renew your passport, you will need:
- Your most recent US Passport
- 2 identical color passport photographs, 2"x2" with a white background (taken no more than 6 months ago)
- A marriage certificate or court order if your name has been changed
- Check or Money Order made out to the US Dept of State
        $75 (regular service)                   Passport Processing Schedule
        $135 (expedited service)

        


Customers who are traveling within 2 weeks (14 days) or who need foreign visas for travel must make an appointment at the National Passport Information Center at 1-877-487-2778, Monday-Friday 8:30am - 4:30pm local time, excluding Federal holidays.
