Vital Records Information & Forms
Vital records (Birth, Marriage and Death Certificates) can be obtained from the City Clerk's Office as follows:
- Walk-in requests. City Hall is open to the public. Please wear a mask upon entering the building.
- On-line requests are typically processed and mailed out 2-3 business days after your request. Requests are processed via Uni-Pay Bank and you can pay with either a credit or debit card. Vital Records purchased on-line are mailed through the USPS. We are unable to expedite your request with an overnight carrier.
- For mailed in requests, please complete the appropriate form(s) below, enclose payment and a self-addressed stamped envelope.
If your request is urgent, we recommend that you send your request by an overnight carrier along with payment and a self-addressed, pre-paid overnight envelope. Please call our office at 978-619-5611 to let us know of your urgent request.
