The following forms and information will help guide all vendors and applicants who are interested in pursuing a license from the City of Salem. All inquiries can be made to the Licensing Board Clerk, Melissa Pagliaro, at 978-619-5631 or via email at mpagliaro@salem.com.
- Amusement Device License Application - Jukebox, pool tables, arcade games, etc.
- Amusement/Entertainment License Weekday - Requirements and application for an Entertainment License. All forms of amusement and entertainment including radio, music in any form, television, juke boxes, etc., require a license. Sunday licenses require a separate application.
